Program Checklist Prior to Operation
Prior to operation, a University Program must meet the following requirements:
- Formally appoint a Program Director
- Only Authorized Adults may interact with Minors.
- Register in the Minor Protection Registration System 30 days prior to start date.
- Wait for Authorized Adult list from Risk Management.
- Obtain participation agreement, approved by Risk Management, and signed by all Minor’s parents/guardians.
- Notify Minors and parents/guardians of BYU Privacy Notice
- Arrange for emergency medical treatment and notification including:
- Identify who will contact emergency medical services in the event of an emergency
- Identify what emergency medical service will be contacted in the event of an emergency (i.e. Provo EMS, BYU EMS etc)
- Establish a procedure for contacting parents in the event of an emergency
- Maintain accessible contact information for parents/guardians
- Maintain accessible contact information for other emergency contacts for all Minors in the event parents/guardians cannot be reached
- Provide parents/guardians information detailing way Minors may be contacted during the event
- Establish age-appropriate procedure for drop-off and pick-up of Minors
- If event includes high risk activitiessubmit a safety plan to Risk Management.
