Program Checklist Prior to Operation

Prior to operation, a University Program must meet the following requirements:

  • Formally appoint a Program Director
  • Only Authorized Adults may interact with Minors.
  • Register in the Minor Protection Registration System 30 days prior to start date.
  • Wait for Authorized Adult list from Risk Management.
  • Obtain participation agreement, approved by Risk Management, and signed by all Minor’s parents/guardians.
  • Notify Minors and parents/guardians of BYU Privacy Notice
  • Arrange for emergency medical treatment and notification including:
    • Identify who will contact emergency medical services in the event of an emergency
    • Identify what emergency medical service will be contacted in the event of an emergency (i.e. Provo EMS, BYU EMS etc)
    • Establish a procedure for contacting parents in the event of an emergency
      • Maintain accessible contact information for parents/guardians
      • Maintain accessible contact information for other emergency contacts for all Minors in the event parents/guardians cannot be reached
  • Provide parents/guardians information detailing way Minors may be contacted during the event
  • Establish age-appropriate procedure for drop-off and pick-up of Minors
  • If event includes high risk activitiessubmit a safety plan to Risk Management.
